Windsor One Call is an independent carecoordinator for patients in need
of health careservices in the home. Providers that participate inthe One
Call program gain access to more cases,while reducing the expense and
hassle associatedwith time-consuming and resource-intensiveadministrative
services. That's right - more casesand less workload!
Designated
Care Coordinator
Windsor provides care coordination and placementservices for patients
of the following entities:
- Hospital discharge planners
- Physician office care coordinators
- Case managers
- Insurers such as HMOs, PPOs, long-term care,and workers compensation
plans
- Once care coordination is designated to Windsor, those in need of
home-based healthcare receive guidance, coordination of care and placement
services.
Comprehensive Network of Providers
Windsor maintains a centralized, easily accessible database of providers
who are participating in the One Call Program. When Windsor is designated
as the care coordinator, we handle all the upfront, labor-intensive services,
then connect patients to providers of home health, infusion, hospice,
DME and medical supply services.
